Hi,

Does anyone know how to add the user avatar beside their thread as seen below?

It can be done by using a plugin called Last post avatar, you can find it here

It's not working properly with the theme:

Any idea what the issue is?

Reason is simple, you need to make css edit to the plugin css file and it should work fine.
